End-faces are inspected before mating, and cleaned or re-terminated when they fail.

First-Article Gate
Each engineer's first panel is tested straight away, on your tester if there is one on site. Volume starts after it passes.

Links tested to the limits your specification names: Tier 1 loss, and Tier 2 with an OTDR where it asks.

Which tester do your engineers use?
The one your specification names. If the site has its own tester, the first-panel test runs on it.
What happens when a link fails?
It is cleaned, re-terminated and re-tested, and the pack carries the passing result under the same cable ID.
